This form is used to request that in-service credit is given for any activities or rainings you complete which are not facilitated by the CASA staff. If you receive a Certificate of COmpletion or Certification of Attendance, please be sure to include a copy. One hour of continuing education credit will be given per 100 pages of material read or per 1 hour of film/video viewed.
